A patient, already known to have chronic myeloid leukemia (CML) and treated with busulfan, was hospitalized because of the appearance of a lymphnode mass in the right inguinal site and lymphedema of the ipsilateral leg. The CT did not show thoracic altarations but abdominal findings included enlarged and conglomerated right inguinal, external common and internal bilateral iliac, paraoltic and paracaval lymph nodes. The hematologic examinations (peripheral blood and bone marrow biopy) confirmed a pattern like that g CML, and biopsy g the right inguinal lymph node mass led to the histologic finding g lymph nodal crisis g CML (Figure 1,2). The myeloblastic nature g the cells replacing lymphoid tissue was confirmed by classic cytochemical and immunocytological methods. The disease, in spite of appropriate therapies, led quickly to the patients death. An exclusively extramedullary blast crisis g CML is not frequent but can involve various districts 1: the pattern described by us has unusual clinical and cytomorphologic features.
